The other form to be used, is the form for fingerprinting information to be done by the Parma Police Department.  Double click the form below, print the form off, fill it out and take it with you to the Police Department when being fingerprinted.

Liability Protection - Ohio Revised Code 121.404 provides liability protection to registered Ohio Citizens Corps volunteers during local, state or federally declared emergencies, disasters, drills and trainings.  The statute also exempts a registered volunteer's personal information on the Ohio Citizen Corp Database from public disclosures.
